RESOLUTION 2003 - 93

A RESOLUTION AUTHORIZING THE RENEWAL OF THE VILLAGE OF VERNON HILLS DENTAL INSURANCE PLAN WITH AMERITAS LIFE INSURANCE COMPANY.

WHEREAS, all professional service engagements, which require the expenditure of over $10,000, require Board of Trustee approval; and

WHEREAS, the Village Board has determined that Ameritas Life Insurance Company is well suited to provide dental insurance coverage for the Village of Vernon Hills Employee Dental Insurance Plan.

NOW THEREFORE BE IT RESOLVED BY THE VILLAGE PRESIDENT AND BOARD OF TRUSTEES OF THE VILLAGE OF VERNON HILLS, LAKE COUNTY, ILLINOIS:

That the Village Manager is authorized to renew the Village of Vernon Hills Employee Dental Insurance Plan with Ameritas Life Insurance Company for the period of June 1, 2003 through May 31, 2004.

That the monthly premiums for the Village of Vernon Hills Employee Dental Insurance Plan be $27.92. for single coverage and $79.64 for family coverage.

That the Village is authorized to pay 75% of the monthly premium or $20.94 for single, $59.73 for family coverage and employees will pay 25% of the monthly premium or $6.98 for single, $19.91 for family coverage for the Village of Vernon Hills Employee Health Insurance Plan, effective June 1, 2003 through May 31, 2004.
